Job Description - Life Coach

•	Help young men ( ages 13-18) find direction and reach their full potential
•	Ability to empower youth and establish realistic plans to achieve individual goals
•	Get to know each of the youth quickly, access their goals efficiently and formulate strategies to help meet their goals
•	Utilize electronic data system to document and track services; (such as Daily Progress Notes and Critical Incident Reports)
•	Utilize various strategies to encourage and motivate youth to meet their goals
•	Dispense residents medications and properly document medications given
•	Adhere to organization’s policies and procedures providing an environment of consistency
•	Understand the importance of safety and how to prevent critical incidents
•	Help to maintain a clean living environment at all times
•	Teach youth appropriate life skills and help work through negative behaviors
•	Transport youth to and from appointments and social events; safely and timely
•	Build positive relationships with youth DAILY; goal of every day.

Job Description - Case Coordinator

•	Empower young men (ages 13-18) and help to establish realistic plans to achieve individual goals; adjust short-term goals and strategies every 30 days
•	Coordinate and monitor health visits including health assessment, dental, physical, medical evaluations and therapy appointments
•	Complete necessary documentation to facilitate referrals to collaborative outside specialty providers to ensure continuity of care (such as child mental/behavioral therapy)
•	Utilize electronic data system to document and track all case management services; (such as Youth Program Plans, Progress and Case Notes, Critical Incident and Monthly Reports)
•	Teach youth appropriate life skills and help work through negative behaviors
•	Monitor progress notes (ie, in document system) daily to ensure notes are entered every day by every shift. When notes are missing will be responsible for ensuring Life Coaches get progress notes entered within 24 hours
•	Monitor progress notes and critical incident reports of Life Coaches to ensure that notes are valid, consistent and compliant
•	Dispense resident's medications and properly document medications given

Job Qualifications - Life Coach

•	Must be at least 21 years of age
•	Must have graduated High School or completed GED
•	Must have a current driver’s license (good driving record)
•	Must pass a KBI and FBI background check
•	Must be able to work with ‘male’ adolescents with behavior problems

Note: A PLUS - 2 or more years of experience working with male adolescents with behavioral type issues or continuing education in the field of Psychology, Social Work, or Criminal Justice.

Job Qualifications - Case Coordinator

•	Must be able to work with ‘male’ adolescents with behavior problems
•	Build positive relationships with youth DAILY; (goal of every day)
•	Ability to make routine decisions and determine proper action based on experience within the limits of organization’s policies and procedures
•	Transport youth to and from appointments and social events; safely and timely
•	Ability to work effectively with people of diverse cultures, ages and economic backgrounds
•	Strong administrative/clerical skills including basic computer skills
•	Understand the importance of safety and how to prevent critical incidents
•	Flexibility to work overtime and some holidays
•	Must be at least 21 years of age
•	Must have at least a BA Degree
•	Must have a current driver’s license (good driving record)
•	Must pass a KBI and FBI background check
•	Must have effective organizational skills and ability to handle multiple activities with changing priorities simultaneously

*A PLUS – Degree in Psychology, Sociology, Social Work, and or Criminal Justice

Job Work Schedule - Life Coach

Week Day 2nd Shift - (Mon-Fri) 2:00pm – 10:00pm
Week Day 3rd Shift - (Sun -Wed) 10:00pm - 8:00am **This is a 10 hour shift
Week Day 3rd Shift - (Thur - Sat) 10:00pm - 8:00am **This is a 10 hour shift

Weekend 1st Shift - (Sat & Sun) 8:00am –4:00pm
Weekend 2nd Shift - (Sat & Sun) 2:00pm –10:00pm
Weekend 3rd Shift - (Sat & Sun) 10:00pm - 8:00am **This is a 10 hour shift
